On July 19, 2012 the Clearfield-Jefferson Community Mental Health Center held a ribbon cutting for the new Integrated Care program.  Partnering with the DuBois Regional Medical Center and Genoa Healthcare, residents in Clearfield and Jefferson counties will be provided “one-stop shop” healthcare services.  The program is designed to provide the individual with mental health, physical health, and pharmacy services under one roof.  DuBois Regional Medical Center will provide the physical health services and Genoa Healthcare, the pharmacy services.  The program is structured to enable a “warm hand-off” of consumers between the providers, thus meeting their immediate healthcare needs.

National statistics indicate that individuals with persistent mental illness die 25 to 30 years early.  The Integrated Care program will address the individual’s total healthcare needs, thus improving their quality and quantity of life.

For additional information on Integrated Care, please contact Christina Martz at (814) 371-1100 xt 296.
